Reporting Form

The Office of Institutional Equity (OIE) receives reports concerning alleged Prohibited Conduct pursuant to the Policy Against Discrimination, Harassment and Related Interpersonal Violence (Policy). This form may be used for all concerns regarding Prohibited Conduct under the Policy, whether the report is regarding an individual’s own concerns, or a report is being made pursuant to the responsible employee reporting responsibilities set forth in the Policy, including those specific to student disclosures of sexual assault, intimate partner violence, and stalking.

Upon submission of this form, OIE will follow up with the reporting party by phone or email. Information provided to OIE is not confidential, but will be handled with discretion. Individuals reporting information to OIE are protected from retaliation under the Policy Against Discrimination and the Non-Retaliation Policy.

If any individual listed in this report form uses a name that is different than their listed name in University records, please indicate this in the first sentence. Also, please do not use pronouns. When referencing individuals, use the last name to avoid any confusion (e.g., Smith entered the room).

Please note that submission of this form does not, on its own, constitute filing a formal complaint. If you wish to file a complaint regarding an allegation of Prohibited Conduct against a University employee, please use OIE’s Discrimination and Discriminatory Harassment Complaint Form.

Finally, please note that intentionally creating a false report is a violation of The Student Code and the employee Code of Conduct.
